Terms and conditions • To avail the offer a Fazaa member must present a valid Fazaa Card • The offers are not valid in conjunction with any other deals or promotions - The parent should fill up Playhouse Registration and Medical Form with all necessary information. - Requested documents for each child file are: - • Copy of Passport, Residence Visa & Emirates ID of the Sponsoring Parents - • Copy of Passport, Residence Visa & Emirates ID of the Child - • Copy of the Child Immunization Record / Vaccination Card of the Child - • Copy of Birth Certificate of the Child - • 2 Passport Size Photos of the Child - • Signed Playhouse Policies, Terms and Conditions - PLAYHOUSE prefers the parents to provide all necessary information about the child that can help to the supervisors and assistants in the beginning so the child can feel comfortable with our employee and nicely settled into the nursery environment. - Attendance Policy - It's always preferable that children attend the nursery on a regular basis. NO REFUNDS / NO FREEZING are made for any missed days (sickness, vacation, holiday, leaving the country, etc.). If child is absent, parents should inform the nursery. If a parent feels the need to remove their child from the nursery, they should give a two-week written notice in order the vacancy to be filled. - Fee Policy - Paid fees are not refundable or transferable in any case. No reductions will be given in case of absence, illness, vacation, holiday or unforeseen closures. Payment can be done by cash, credit card or cheque. Fee invoices will be sent to the parents at the beginning of each term and needs to be paid in full within a week. If fees remain unpaid by the end of the second week of the trimester, the child will no longer be admitted to attend until accounts are settled. - Do not send a child who has these common signs or symptoms of illnesses: - • Fever - • Sore Throat - • Persistent cough or sneezing - • Runny Nose - • Vomiting or Diarrhea in the past 24 hours - • Ear Infection - • Rashes - • Eye Infection - • Head Lice - • Chicken Pox - If your child has been prescribed antibiotics, may return back to class 24 hours after the first dosage.
